PART 1: Please fill out one form per breeding. Fee $30.00 (USD). Must be filled out after breeding and before whelping. This will nominate the bitch and the whole litter. Individual puppy nominations still required at no additional fee.

Bitch must be nominated before whelping.
The bitch must be owned or leased by an active member of the PPPA.
The form must be returned to the Futurity Chairman no later than midnight prior to whelping. The determination date for timely nominations shall be the online submission date.
The nomination of the litter qualifies any of the litter for entry in the futurity stakes, regardless of the ownership of the individual puppy when entered or shown. Each individual puppy must be nominated. ALL NOMINATIONS MUST BE MADE BEFORE THE PUPPY REACHES FOUR MONTHS OF AGE, ON AN OFFICIAL PUPPY NOMINATION FORM PROVIDED BY THE FUTURITY CHAIRMAN AFTER THE LITTER IS NOMINATED.
If the breeder wishes to nominate all the puppies in the litter individually, this may be done at any time after the date the litter is whelped but before it is four months old.
